"I.F.P.C.C. has a published and long standing membership position with regards to Professionalism and Public Service. Because we are Ambassadors of God and accountable first and foremost to Him, it is imperative we uphold the behavioral Code of Ethics under which we were sworn into the Office of Chaplain. This Code of Professional Conduct does not allow us to compromise our spiritual, civic, moral or ethical duty in any manner. We must diligently avoid even the appearance of evil as Scripture advises. Therefore, I.F.P.C.C. conducts itself as a unique organization, separate from other organizations, accountable to God, with ministry, field work and public service conducted according to our Code of Ethics".

Page one is for first time membership application, membership renewal and Chaplaincy renewals only. Page two is an Ordination/Ministerial license renewal form for returning Ordained Ministers only.
